POSSO believes that all members of the community are entitled to a productive, secure, and rewarding life, and that good health is an essential component. The promotion and maintenance of a healthy lifestyle has proven to be the most urgent concern. Toward this end, POSSO provides, through a public health nurse and medical professionals, health education, blood pressure screening, eye exams, and other medical screening, consultation, and referral.
Blood Pressure and Medical Screening
The blood pressure and medical screening provided by a public health nurse is one of our most-utilized programs. Participants agree that this program is vital to the maintenance and restoration of their health, and in ensuring timely treatment of potentially-serious health problems.
POSSO continues to provide case management for individuals with multiple serious problems or who have difficulty negotiating the complex health and human services system. These individuals need extra support to live independently. These services are supported in part by the United Way of Santa Clara County and the City of San Jose.
For those with serious problems in the areas of mental health, family or individual functioning, counseling services are provided by professional clinical social workers having the special cultural and language expertise to facilitate problem solving.
Health and nutrition workshops are sponsored throughout the year with the collaboration of health organizations and medical professionals.
